Advancements in information technology have significantly influenced various sectors, including healthcare services, particularly in improving the effectiveness and efficiency of data management and administrative processes. Puskesmas Cogreg, as one of the primary healthcare facilities, still faces several challenges in the internal goods ordering and procurement process, which is currently carried out manually. These issues include poorly organized data recording, the risk of document loss, and delays in the submission and approval process of goods requests. This study aims to design and develop a web-based online form application to support the internal goods ordering and procurement process at Puskesmas Cogreg. The system development method applied in this research is the Waterfall method, which consists of requirements analysis, system design, implementation, testing, and maintenance stages. The developed system is equipped with several main features, including goods request submission, verification processes by authorized parties, and real-time monitoring of procurement status. The results of the study indicate that the developed application is capable of improving the efficiency of goods procurement management, reducing data recording errors, and accelerating the approval process for goods requests. In addition, the system supports a more structured, transparent, and well-documented administrative process within Puskesmas Cogreg.
